Referral Process — Frequently Asked Questions

Sunbridge supports patients requiring skilled nursing, private duty nursing, infusion services, and complex care management. We work with pediatric and adult patients with medical, behavioral, and chronic care needs who are appropriate for in-home services.

To initiate a referral, we typically request a clinical summary, plan of care, physician orders, current medication list, insurance information, and patient or caregiver contact details. Our intake team will promptly notify you if any additional documentation is needed.

Referrals are reviewed promptly upon receipt. In most cases, our intake team will contact the patient or caregiver within one business day and provide updates to the referring provider as the referral progresses.

Sunbridge works closely with referring physicians to implement and maintain the plan of care. We manage physician orders and medication changes, communicate clinical updates as needed, and promptly notify providers of any changes in patient condition or care requirements.

Sunbridge participates with multiple commercial, Medicaid, and managed care plans. Our intake team verifies coverage during the referral process and will communicate any authorization requirements or limitations.

Sunbridge utilizes a streamlined submission process with Alabama Medicaid. In most cases, approvals are obtained within approximately seven days, allowing services to begin as quickly as clinically appropriate.

Referring providers may contact our intake team directly by phone or secure fax or email. We are happy to provide referral status updates and next steps at any point in the process.
